Outreach has an opening for a full-time Registered Nurse to provide and/or coordinate health services and health-related-care to residents of our three Brentwood, Long Island locations.
What you will own
Noble Hearts HR Consulting Inc., is the talent sourcing partner for Outreach. Our client Outreach is seeking a qualified candidate for the position of PT Registered Nurse to work in our REACT Center located in Nassau County. The Registered Nurse is an integral part of the treatment team who provides health care services for individuals in their care, ensuring that all overall health and safety needs are met. This position works under NYS Office of Mental Health OMH regulations. This position is 19 hours a week and there is some flexibility regarding day and time. The schedule for this position is: Monday - Thursday 5PM - 9PM; Friday 10AM - 2PM.

Are you passionate about providing personalized medical care and making a positive impact on individuals' health and well-being? React Center is seeking a dedicated nurse to conduct comprehensive medical assessments and deliver exceptional patient care in a supportive team environment.
Key Responsibilities:
- Holistic Assessments: Perform thorough medical assessments, including gathering health histories and determining the need for physical exams or medical services tailored to individual needs.
- Medication Administration: Administer medications, including intramuscular injections, as prescribed by our experienced medical team.
- Patient Care Coordination: Follow up on health-related referrals and maintain a robust tracking system to monitor patient health issues, appointments, and medications.
- Patient and Staff Education: Provide valuable patient education and counseling on health issues while also conducting educational sessions for staff on pertinent health-related topics.
- Crisis Intervention: Offer immediate crisis intervention during emergencies to ensure patient safety and well-being.
- Efficient Documentation: Complete all necessary documentation and reports promptly and accurately, ensuring seamless communication with Program leadership regarding any challenges or anticipated deadlines.
- Team Collaboration: Actively engage in staff meetings, case conferences, and unit meetings, fostering effective communication and teamwork with clinical staff to enhance our integrated clinical services.
- Health Needs Assessment: Identify and address current health needs within our patient population, implementing tailored activities to bridge service and informational gaps effectively.
